The Effect of Cligel? (Hydrogel Sheet) Dressing in Split Thickness Skin Graft Donor Site

Abstract
In Korea, we applied the moist dressing using the polyurethane foam which was developed in domestic market in 2002. This changed the dressing concept from dry dressing to moist dressing. Foam dressing is a very useful dressing for absorbing the adequate amount of exudation and keeping the wound in moisturized environment. But it was very difficult to develop the dressing material that kept the wound in moisturized state and reduced pain, but moreover developing such a dressing material that contained the healing growth factor. So we did not stop at just dressing the wound, but started to concentrate on dressing material like hydrogel which could contain the healing growth factor. We examined how effective the Poly-N-vinylpyrrolidone hydrogel sheet (with abberviation PVP) namely Cligel? is for wound healing. The structure of Cligel? is composed of the PVP hydrogel sheet and polyurethane film which could penetrated the air but impermeable to the water and microoraganism such as bacteria, so it could prevent the infection from external. We performed the half-side test: 15 patients who have the split thickness of skin graft and admitted in plastic surgery department of Seoul National University from October 2004 to March 2005 and 20 patients who admitted in plastic surgery department of Yang-Ju Hospital. With this test, we evaluated and compared the healing effectiveness, validity for using (Functional effect) and comfort which was felt by patients when they changed the dressing material between those two dressing materials (wound effect). The test was evaluated by points according to Wilcoxon signed rank and correlated the statistics. According to the half-test results on split thickness of skin graft, complete wound healing date of Foam dressing was 9.7?1.6 days and Cligel? was 9.1?0.9 days. It means Cligel? has short healing period and better healing ability. Validity for using (Functional effect) was 5.4?1.1 points for the Foam dressing and 4.7?1.7 points for Cligel?. With this result, we could see the Foam dressing has better validity than hydrogel. The comfort of patient when the dressing material put on wound(wound effect) was 4.9?2.1 points for the Foam dressing and 5.5?0.7 points for Cligel? so we could say that the hydrogel is more comfortable to patients. So we concluded that the occlusive dressing with Cligel? is an effective dressing method in split thickness skin graft donor site. And we expect that it is also an effective dressing material in many other wounds. (J Korean Wound Care Soc 2005;1:78-82)
